Conversion Procedure

Applicants who are interested in embracing Islam must disclose their home address. This is important so as to ensure that the converted applicant is registered under the State Department of Islamic Religion. Applicant will be told about the:

a) 5 Pillars of Islam
b) 6 Pillars of Faith
c) Islamic jurisprudence
d) Declaring the Syahadah

Islamization procedure at the Federal Territories Department of Islamic Religion (JAWI)

1. Applicants need to provide the following documents and items:

a) Identification card (local citizen)
b) Passports and visas (foreign nationals) must exceed one month before from the expiry date
c) 2 photos (passport size)
d) 2 Malaysian Muslim male witnesses
e) The applicant must submit the conversion form to JAWI after completing the conversion at PERKIM.
The two (2) witnesses are required to accompany the applicant to JAWI to do the next registration.

Islamization procedure at the Selangor Islamic Religious Council (MAIS) Majlis Agama Islam Selangor - MAIS

  1. Applicants need to provide the following documents and items:
  2. a) Identification card (local citizen)
  3. b) Passport and visa (foreign nationals)
  4. c) 4 photos (passport size)
  5. d) 2 Malaysian Muslim male witnesses

IDENTITY CARD AMENDMENT AFTER CONVERSION TO ISLAM DOCUMENTS REQUIRED (Original and Copies) :

1) Applicant’s Identity Card

2) Card certifying embracing of Islam from the Islamic Religious Department or State Islamic Religious Council

3) Birth Certificate or Adoption Certificate or Citizenship Certificate or confirmation of citizenship status or entry permit (as applicable)

4) Statutory declaration (if the applicant is under 18, the statutory declaration must be made by the parent or *guardian. *Guardian can be the grandfather, grandmother, aunt, uncle, elder sister or brother who is over 21.)

5) Application Form JPN.KP16 (obtainable from the NRD counter) that is completed.
